Abstract:

Introduction: Global health systems face a growing burden of chronic non-communicable diseases and persistent gaps in equitable access to medical services. Against this backdrop, optimizing Primary Health Care (PHC) is essential. This presentation analyzes the role of Family Medicine as the fundamental structural pillar for ensuring the sustainability of the health system and mitigating disparities in community clinical outcomes.

Objectives: To evaluate the impact of preventive programs led by Family Medicine specialists on reducing costs and morbidity, and to propose a framework based on systematic clinical screening that effectively promotes health equity in vulnerable communities.

Methodology: A reflective analysis and comprehensive document review were conducted regarding the main clinical prevention strategies and institutional guidelines implemented in the Ibero-American region in recent years. Key indicators—such as screening coverage, hospitalization rates for Ambulatory Care-Sensitive Conditions (ACSCs), and the scope of community-based care interventions—were examined.

Results: Evidence demonstrates that care models focused on clinical prevention and timely screening not only significantly reduce the rate of complications associated with chronic conditions but also serve to mitigate socioeconomic inequities. By decentralizing care and integrating it directly into the family and community setting, healthcare resources are substantially optimized, thereby easing the strain on emergency and high-acuity services.

Conclusions: Strengthening Family Medicine and designing preventive strategies with a territorial focus represent the most efficient path toward building resilient, equitable, and economically viable health systems on a global scale.
