Abstract:

The growing prevalence of chronic diseases, particularly among aging and socioeconomically disadvantaged populations, continues to drive increased healthcare utilization and expenditures. This perspective-based manuscript explores the role of Information and Communication Technologies (ICT) in the management of chronic conditions, with a primary emphasis on older adults. A range of digital health tools including electronic health records (EHRs), mobile health (mHealth) applications, telehealth and telemedicine platforms, wearable devices, remote patient monitoring systems, and clinical decision support systems are examined in terms of their utility, outcomes, and implementation challenges. The analysis highlights the potential of these technologies to enhance disease monitoring, promote patient self-management, reduce hospital readmissions, and improve overall care coordination. Key benefits include real-time data sharing, improved treatment adherence, and increased patient engagement. However, persistent barriers to adoption such as digital literacy gaps, concerns regarding data privacy, infrastructural limitations, and disparities in broadband access are critically reviewed. Despite these challenges, the integration of ICT into chronic disease care has demonstrated meaningful improvements in clinical outcomes and patient empowerment. The manuscript concludes by emphasizing the need for targeted research, user-centered design, and supportive policy frameworks to fully realize the transformative potential of digital health in chronic disease management.
